Renal colic – Information for GPs

Referral criteria / commissioning position

Referral to secondary care if:

  • primary care intervention and management fails to resolve symptoms or condition
  • red flag symptoms identified or pain not controlled – consider emergency admission to Urology
  • pain is unmanageable or patient is positive for stones or Hydronephrosis – refer for Urgent Urology & CT Urogram

Red flag symptoms

  • Solitary functioning kidney
  • Pyrexia +/- other signs of infection (such as peritonitis)
  • Systemically unwell (hypovolaemia) +/- oliguria/anuria
  • Age >60 yrs. especially if no social support

Investigations prior to referral

  • Urinalysis
  • BP/pulse
  • Respiratory Rate
  • Temperature

Information to include in referral letter

  • Details of treatments and measures tried including outcomes
  • Relevant past medical/surgical history
  • Drug history (prescribed and non-prescribed)
  • Relevant past medical/surgical history
  • Current regular medication
  • BMI
  • Smoking status
  • Alcohol consumption
