Humber Teaching NHS Foundation Trust has recently been awarded a contract to deliver the Scarborough and Ryedale Community Care and Support Service.
It will be responsible for delivering some community health services, such as Community and District Nursing and therapy services such as Physiotherapy and Occupational Therapy.
These services must be linked and coordinated with other health, social care and voluntary services, who must work together to prevent people from becoming unwell.
The Humber Trust and NHS Scarborough and Ryedale Clinical Commissioning Group want it to be a service that is developed with patients, carers and partners.
The Trust has put together a survey to find out more about what people think of community services and the new model or care it is adopting.
